Social Inclusion Team (SIT)

The Social Inclusion Team facilitates knowledge of and access to health services for excluded groups and service users across Liverpool. They liaise with a range of professionals and across agencies to support vulnerable individuals and families.


How do we deliver our services?

  • Outreach work for people having difficulties accessing primary care
  • Link Worker service engaging with individuals
  • Advising, training and guiding health professionals
  • Acting as advocates for individual patients
  • Partnership working with other agencies
  • Signposting patients and health professionals to appropriate services
  • Providing a core of expertise to support professionals, individuals, communities and organisations
  • Advising on culturally sensitive service delivery aided by team representation from Arabic, Bangladeshi, Chinese, Irish, Polish, Pakistani, Russian, Somali, Yemeni and the deaf community.

What we don’t do


We cannot provide one-to-one interpretation or translation, but we can signpost you to appropriate services.
