Liverpool Health Trainer Service
A Liverpool NHS Health Trainer is someone who
can support and motivate you to change certain
aspects about your lifestyle or behaviour. They
live and/or work in your local community and understands the issues and challenges you face and are easily accessible and available at times to meet your needs.
All Health Trainers have undergone intensive training and keep up to date with the latest local and national health information
There are lots of things that you can do to live a healthier and longer life. Small changes can often make big differences. Whether you start eating healthier or exercising more, stop smoking or reduce your alcohol intake, cut back on your stress levels or a combination of things - making changes to your everyday routine can help improve your physical and emotional wellbeing.
Health Trainers meet with you in private, at a time convenient for you. They can meet you in a local community venue, GP surgery or at their local office.
Health Trainers will complete a lifestyle assessment. Together you will identify the things that you are currently doing that have a negative affect on your health, and the things that you could start doing to improve your health. This will help you start thinking about your health, your behaviours and what changes you most want to make.
The results will be used to create your own personal health plan that sets out realistic goals and makes sure you don’t try to take on too much at once. Your Health Trainer will offer support throughout the plan and will encourage and motivate you. They can also point out local facilities and services that can be used to make the changes to your lifestyle a permanent and enjoyable part of your life.

Eligibility for the service: You must be at least 16 years of age to access the service and be a Liverpool resident or work in Liverpool
